| 摘 要：［目的］探讨肺癌患者住院费用的变化和影响因素，为肺癌患者住院费用合理控制提供理论依据。［方法］ 选取2002~2014年湖南省某三级甲等专科医院收治的1542例肺癌患者为研究对象，采用方差分析和多元逐步回归分析对其多次住院总费用影响因素进行分析。［结果］1542例肺癌患者平均就诊年龄56.49±9.7岁，平均住院天数33d，人均住院费用34 300.0元，且2002~2014年总体呈上升趋势；住院费用构成中，药品费占比最大，达56.6%，其次是材料费、检查费和治疗费，其占比分别是9.7%、8.4%和8.1%。住院费用的影响因素依次为：是否手术、住院次数、住院天数、伴随疾病否、就诊年龄、是否腺癌和是否放疗，多元回归模型中均有统计学意义（F=301.40，P＜0.05）。［结论］ 2002~2014年间肺癌患者住院费用总体呈上升趋势，建议医院针对费用影响因素，加强对药物科学合理使用、缩短住院天数及进一步提高医务工作者的医疗水平，以有效控制肺癌患者医疗费用的增长，减轻其医疗负担。 |

| Abstract：［Purpose］ To analysis the component feature and variation trend of patients with malignant tumor during 2005~2014 in a cancer hospital of Xinjiang. ［Methods］ The data of malignant tumor patients admitted in a cancer hospital of Xingjiang from 2005 to 2014 were collected. The disease constitution，demographical features of patients and the variation trend of malignant tumor were analyzed by SPSS 13.0 statistical software. ［Results］ Total 70 279 cancer patients were admitted during 2005~2014 in this hospital. The top 10 malignant tumors were breast cancer，lung cancer，cervical cancer，stomach cancer，esophageal cancer，colorectal cancer，liver cancer，thyroid carcinoma，lymphoma，bone and soft tissue tumors. The average growth rate of admitted cancer patients was 6.13%. There were 29 825 male patients (42.45%) with a average growth rate of 5.15%；and 40 454 female patients，(57.55%) with an average growth rate was 6.63%. The growth rate was over 6.13% for patients in age group 30~，40~，50~ and above 70. There was a slight difference in the growth tendency of case number between gender and age groups. ［Conclusion］ There is a steady increase in the number of the total malignant tumor cases；the disease composition，sequence，and high-incidence age are different between males and females. |
