| 摘 要：［目的］ 分析2019年浙江省海宁市上消化道癌机会性筛查的效果，以期为上消化道癌防治提供科学依据。［方法］ 选取2019年6—12月来海宁市中医院参加上消化道癌机会性筛查的40~74岁人群为研究对象，对其进行消化道内镜检查及指示性病理活检，计算筛查顺应性、阳性病例检出率和早诊率，同时开展卫生经济学评价。［结果］ 共确定1256名筛查目标人群，其中1114例完成消化道内镜检查，筛查顺应性为88.69%。共检出阳性病变67例，检出率为6.01%，其中早期病变60例，早诊率为89.55%。共投入筛查成本为424 626.68元，检出1例早期病变的成本为7077.11元，以海宁市人均GDP计算早期发现成本系数为0.05。［结论］ 上消化肿瘤机会性筛查顺应性高，筛查的效果和效益较好，适合更大范围推广。 |

| Abstract：［Purpose］ To analyze the effectiveness of opportunistic screening for upper gastrointestinal cancer in Haining City in 2019. ［Methods］ Patients aged 40 to 74 years who participated in opportunistic screening of upper gastrointestinal cancer while visiting the hospital from June to December 2019 were included in the study. Gastrointestinal endoscopy and biopsy were performed for the screening. The screening compliance，positive detection rate and early diagnosis rate were calculated，and health economics evaluation was carried out. ［Results］ A total of 1256 patients were identified for the screening，among whom 1114 subjects completed gastrointestinal endoscopy with a screening compliance rate of 88.69%. Positive lesions were detected in 67 cases with a detection rate of 6.01%，including 60 cases of early lesions with an early diagnosis rate of 89.55%. The total cost of screening was 424 626.68 Yuan，and the detection of one early case costed 7077.11 Yuan. Based on the per capita GDP of Haining City，the early detection cost index was 0.05. ［Conclusion］ The opportunistic screening of upper gastrointestinal cancer in Haining City has high compliance，good screening effect and benefit，which is worth of promotion. |
