      摘 要:[目的] 分析2018年甘肃省肿瘤登记地区甲状腺癌发病和死亡情况及2010—2018年甲状腺癌变化趋势。[方法] 按照国家癌症中心制定的数据质量审核评价标准,对甘肃省符合质量控制标准要求的15个肿瘤登记点上报的甲状腺癌发病、死亡和人口数据进行汇总分析,分性别和城乡计算发病(死亡)率、标化发病(死亡)率、0~74岁累积率等统计指标,中标率及世标率分别采用2000年全国普查标准人口年龄构成和Segi’s世界标准人口年龄构成计算。使用Joinpoint线性回归模型计算2010—2018年甲状腺癌发病率和死亡率的年度变化百分比(APC)。[结果] 2018年甘肃省肿瘤登记地区甲状腺癌新发病例数为213例,发病率为3.96/10万,中标率为3.20/10万,世标率为3.18/10万,累积率(0~74岁)为0.31%,占全部恶性肿瘤发病的1.57%,居发病顺位第16位。甲状腺癌死亡病例数为37例,死亡率为0.69/10万,中标率为0.50/10万,世标率为0.57/10万,累积率(0~74岁)为0.05%,占全部恶性肿瘤死亡的0.49%,居死亡顺位第19位。发病率和死亡率分别在20岁和45岁开始随年龄增长呈上升趋势,高峰分别在50~54岁和80~84岁年龄组。2010—2018年甘肃省甲状腺癌中标发病率从2010年的2.62/10万上升至2018年的3.20/10万(APC=2.08%,95%CI:-6.71%~11.69%),趋势变化差异无统计学意义(P>0.05);中标死亡率呈下降趋势(APC=-10.05%,95%CI:-17.97%~ -1.35%),趋势变化差异有统计学意义(P<0.05)。地区分布分析显示,景泰县甲状腺癌中标发病率最高,临潭县中标死亡率最高,高台县中标发病率与死亡率均为最低。[结论] 2018年甘肃省肿瘤登记地区甲状腺癌发病率虽处于我国较低水平,但其存在一定的上升趋势,应加强开展健康宣传教育,控制危险因素,有针对性开展综合防治工作。

      Abstract: [Purpose] To analyze the incidence and mortality of thyroid cancer in cancer registration areas of Gansu Province in 2018 and the trend from 2010 to 2018. [Methods] According to the data quality review and evaluation standards formulated by the National Cancer Center, the thyroid cancer incidence, mortality and population data reported by 15 cancer registries in Gansu Province that met the quality control standards were analyzed. The incidence(mortality) rate, age-standardized incidence(mortality) rate, and cumulative rate(0~74 years old) were calculated according to areas(urban and rural) and age groups. The age-standardized rate was calculated and adjusted by Chinese standard population in 2000(ASIRC, ASMRC) and Segi’s world standard population(ASIRW, ASMRW), respectively. Using the Joinpoint linear regression model to analyze and calculate the annual percentage change(APC) of the incidence and mortality of thyroid cancer in Gansu Province from 2010 to 2018. [Results] In 2018, the crude incidence of thyroid cancer in the cancer registration area of Gansu Province was 3.96/105, ASIRC and ASIRW were 3.20/105 and 3.18/105, respectively, and the cumulative rate (0~74 years old) was 0.31%, accounting for 1.57% of all new cancers, ranking the 16th in the order of incidence. The crude mortality rate was 0.69/105, ASMRC and ASMRW were 0.50/105 and 0.57/105, respectively, and the cumulative rate(0~74 years old) was 0.05%, accounting for 0.49% of all cancer deaths, ranking the 19th in the order of mortality. Incidence and mortality began to increase with age at 20 and 45 years old, respectively, and peaked in the 50~54 and 80~84 age groups, respectively. From 2010 to 2018, the ASIRC of thyroid cancer in the cancer registration areas of Gansu Province increased from 2.62/105 in 2010 to 3.20/105 in 2018(APC=2.08%, 95%CI: -6.71%~11.69%), the trend change was not statistically significant(P>0.05); the ASMRC of the thyroid cancer showed a downward trend(APC=-10.05%, 95%CI: -17.97%~-1.35%), and the trend change was statistically significant(P<0.05). In the regional distribution, Jingtai County had the highest incidence rate, and Lintan County had the highest mortality rate; Gaotai County had the lowest incidence and mortality rates. [Conclusion] Although the incidence of thyroid cancer in the cancer registration areas of Gansu Province in 2018 was at a relatively low level, and increasing trend of thyroid cancer incidence was observed. Health promotion and education should be strengthened, risk factors should be controlled, and comprehensive prevention and treatment work should be carried out.
