      摘 要:[目的] 分析中国2019年各地区主要恶性肿瘤的死亡特征与空间聚集性,并探索1990年和2019年不同省(区、市)恶性肿瘤死亡率的时空变化特征。[方法] 基于2019年全球疾病负担研究数据,分析不同地区、省(区、市)常见恶性肿瘤标化死亡率及伤残调整寿命年(disability-adjusted life years,DALY)率,并采用相对变化率量化时间变化趋势,以及局部空间自相关分析和热点分析方法探索标化死亡率的空间聚集性。比较不同癌种1990年和2019年的分省(区、市)标化死亡率的变化特征。[结果] 2019年不同癌种标化死亡率和标化DALY率的地区分布基本一致,恶性肿瘤标化死亡率在东北地区最高(159.75/10万),华南地区最低(119.72/10万)。2019年,肺癌、肝癌的标化死亡率在东北地区最高;胃癌在西北地区标化死亡率最高;结直肠癌、食管癌在西南地区标化死亡率最高。肺癌、胃癌、结直肠癌、食管癌、肝癌标化死亡率最低的地区则分别是西北、华南、西北、东北、华东。2019年标化死亡率排名前5的恶性肿瘤高低聚集和热/冷点分别发生在不同省(区、市),与各省(区、市)标化死亡率结果保持一致。与1990年相比,2019年全国各省(区、市)胃癌、食管癌、肝癌的标化死亡率均大幅度下降。结直肠癌标化死亡率在各省(区、市)均有所上升,上升幅度最大和最小的省(区、市)分别是广西和北京。除上海、北京、天津、广东、吉林外,肺癌标化死亡率在其他省(区、市)标化死亡率不同程度上升,增幅最大省份为四川。[结论] 中国不同恶性肿瘤在不同地区、省份的标化死亡率和DALY率存在差异。应持续宣传健康生活方式,根据当地恶性肿瘤情况进行危险因素暴露控制,并尽可能投入资源开展筛查防控。

      Abstract: [Purpose] To analyze the regional difference and spatial clustering of major malignant tumors in China in 1990 and 2019. [Methods] Based on the data from the Global Burden of Disease Study 2019(GDB 2019), the age-standardized mortality and disability-adjusted life years(DALY) rates of common malignant tumors in different regions(provinces, municipalities and autonomous regions) were analyzed. The temporal trends and the spatial distribution and aggregation of age-standardized mortality rate in 2019 were analyzed using relative change, local spatial autocorrelation methods, and hotspot analyses. [Results] The regional distribution of age-standardized mortality rates(ASMR) of various malignant tumors was basically consistent with that of age-standardized DALY rate, that was highest in Northeast China(159.75/105) and lowest in Southwest China(119.72/105) in 2019. In 2019, the ASMR of lung cancer and liver cancer was the highest in Northeast China, stomach cancer in Northwest China, and colorectal cancer and esophageal cancer were the highest in Southwest China. The lowest ASMR of lung cancer, stomach cancer, colorectal cancer, esophageal cancer and liver cancer occurred in Northwest China, South China, Northwest China, Northeast China and East China, respectively. In 2019, the high and low clusters and hot/cold spot of the ASMR of the top five malignant tumors occurred in different provinces, which were consistent with the ASMR of each province. Compared with 1990, the ASMRs of stomach cancer, esophageal cancer and liver cancer in all regions were decreased significantly in 2019. The ASMR of colorectal cancer increased in all regions, with the highest increasing in Guangxi and lowest increasing in Beijing, respectively. Except for Shanghai, Beijing, Tianjin, Guangdong, Jilin, the ASMR of lung cancer increased to various degrees in other regions, with the highest increase in Sichuan. [Conclusion] There are differences in the age-standardized mortality rate and age-standardized DALY rate of different malignant tumors in different regions in China.
