      摘 要:[目的] 分析2013—2017年南通市肝癌发病(死亡)的流行趋势和空间分布。[方法] 利用Joinpoint软件对肝癌的发病和死亡进行趋势分析。利用Geoda软件进行空间自相关分析,结合Moran指数绘制肝癌空间分布聚类地图。[结果] 2013—2017年南通市肝癌粗发病率和粗死亡率分别为48.51/10万和41.02/10万;标化发病率和标化死亡率分别为24.26/10万和19.87/10万。粗发病(死亡)率和标化发病(死亡)率均呈逐年下降趋势(P均<0.01)。男性肝癌的发病率和死亡率均显著高于女性(P均<0.01)。空间自相关分析显示,肝癌的发病与死亡呈聚集性分布(Moran’s I>0,P<0.01),发病、死亡热点区域主要集中在启东市和海门区两地的乡镇,崇川区、经济技术开发区和通州区等城区周边呈低发聚集。[结论] 2013—2017年,南通市各乡镇(街道)肝癌的发病率与死亡率存在明显的空间聚集性,并具有稳定的高发和低发区域,建议采取有针对性的措施,进一步加强肝癌的区域性综合防治。

      Abstract: [Purpose] To analyze the epidemiological trend and spatial distribution of liver cancer incidence and mortality rates in Nantong City from 2013 to 2017. [Methods] The liver cancer incidence and mortality data from 2013 to 2017 were collected from Nantong cancer registries. Joinpoint software was used to analyze the incidence and mortality rates trends of liver cancer. Geoda was used for spatial auto-correlation analysis, and a spatial distribution cluster map was drawn using Moran’s I. [Results] From 2013 to 2017, the liver cancer incidence and mortality rates were 48.51/105 and 41.02/105, respectively; the age-standardized incidence and mortality rates were 24.26/105 and 19.87/105, respectively. The crude and age-standardized incidence and mortality of liver cancer showed a downward trend(both P<0.01). The liver cancer incidence and mortality rates in male were significantly higher than those in female(both P<0.01). The spatial auto-correlation analysis showed that the incidence and mortality rates of liver cancer in Nantong were clustered (Moran’s I>0, P<0.01), mainly concentrated in Qidong and Haimen towns. In contrast, Chongchuan District, Economic and Technological Development Zone and Tongzhou District were areas with lower incidence and mortality rates. [Conclusion] There is a spatial aggregation distribution of liver cancer incidence and mortality rates in Nantong from 2013 to 2017 with stable high or low incidence areas. It is suggested to take targeted measures for comprehensive prevention and control of liver cancer in different regions of Nantong.
